First Visit
Your appointment will consist of a consultation explaining your diagnosis and treatment options with treatment possible the same day as the consultation depending on how the appointment is initially scheduled. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- The referral slip from the referring dental office.
- Picture ID of the patient and/or the responsible party
- A list of medications the patient is presently taking.
- If there is applicable dental insurance coverage and if they have provided you with an insurance card, bring the card with you. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or guardian must accompany all patients under 18 at any visit.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, pregnancy, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics for pre-med.)
X-Rays
We understand you may have had recent x-rays done at your general dentist’s office. You may bring these with you or have them forwarded to our office. We appreciate any additional information that may aid us in your treatment. However, it is our standard practice to take select x-rays of the area for which you have been referred.
