To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. Please provide your insurance information to our staff prior to your initial visit with us so we may more accurately answer any questions you may have. This list of information will allow our staff to correctly identify your policy, correctly bill your services, and expedite your claim payment. Please be prepared with the following information:
- Insurance company hosting your policy (not the local union name and number, but the insurance co. itself)
- Policy holder first and last name, date of birth, insurance ID and/or social security number
- Patient (if not the policy holder) first and last name, date of birth.
Some insurance companies require our office to ask for additional information, such as the zip code associated with the account, the state which manages the insurance coverage and the insurance company phone number. This information may be found on the back of your insurance card.
We are in-network (contract providers) for most major insurance companies and unions, for both PPO and DHMO plans. Unfortunately, we are not medicaid providers at this time. If you would like to confirm if we are providers for your network plan please ask our capable staff.
